Patient Responsibilities
Accurate Information

Appointment Compliance
You are responsible for attending scheduled appointments on time. Cancellations must be made at least 24 hours in advance. Repeated no-shows may result in discharge from our practice.
Treatment Compliance
You must follow prescribed treatment plans, take medications as directed, and attend follow-up appointments. Non-compliance may affect treatment outcomes and future care decisions.
Payment Obligations
You are responsible for all charges incurred for services provided, including co-payments, deductibles, and non-covered services. Payment is due at the time of service unless other arrangements have been made.
